129 West 67th Street will be a pop-up vaccination site this Tuesday, November 9th from 12:30-4pm. Students ages 5-11 must be accompanied by a parent or legal guardian, or by an adult designated by the parent/guardian, while receiving their vaccine. If the student is accompanied by an adult who is not the parent or guardian, the vaccine provider must either obtain verbal consent from the parent/guardian by phone at the time of vaccination, or, parent/guardians may instead choose to send a completed consent form with the adult designee that you can access HERE. Mention “Special Music School PTA” on your registration and, as a partner organization, the PTA will receive $100 for every first dose vaccine administered.
